Use this form to tell HM Revenue & Customs (HMRC) if you are stopping self-employment, closing or selling your business or have never been self-employed.

Please note: HMRC are unable to accept a date earlier than 7 days before the date you stopped self-employment.

You can complete this form on behalf of your client if you are an authorised agent. An authorised agent is one where HMRC has been given authorisation using either form 64-8 or the online authorisation service.
